Diabetics spend a significant amount of time monitoring their blood sugar, watching what they eat and when they eat, and making sure they have access to health care and medicine. The nature of diabetes and the effects the disease can have on a diabetic’s life require diligence and almost constant awareness to ensure it is managed properly.
According to the Centers for Disease Control and Prevention, more than 32 million U.S. adults have been diagnosed with diabetes, and the number of adults diagnosed with the condition in the past 20 years has more than doubled. The disease has a huge impact on a person’s overall health and wellness. Diabetes is the No. 1 cause of kidney failure, lower-limb amputations and adult blindness, and it is among the top 10 causes of death in the U.S.
Effective July 1, Blue Cross Blue Shield’s Blue Care Network of Michigan is offering a diabetes management program powered by Livongo. Its purpose? To help diabetics simplify diabetes management, improve their health, save money and spend more time enjoying life.
Diabetes management can be complicated, stressful, expensive and full of hassles. There is a constant need to organize test strips and medication and regulate blood sugar, and diabetics haven’t always had adequate health care support when they need it.
The Livongo program is designed to remove the barriers that can make it problematic for diabetics from controlling their condition. The program simplifies diabetes management by offering devices including advanced “smart” blood glucose meters that share information with Livongo via a mobile app. Livongo in turn provides customized insights and alerts. Users can also use the devices to share information with their health care providers and loved ones.
Livongo also provides personalized expert coaching and other support. The Livongo Expert Coach facilitates education, provides feedback and support, and ensures diabetics have the tools they need to reach their goals. Coaches are available 24 hours a day, seven days per week, 365 days per year.
Diabetics also have access to unlimited supplies of test strips and lancets, at no cost and shipped directly to health plan members’ homes.
Livongo is using data and analytics to drive behavioral change and give users a personalized experience. Everyone’s struggle with diabetes is unique, and analytics and data based on a person’s health history are essential to the best possible management of the disease.
