Primary care continues at Bradford Medical Park
Primary care remains available at the former Bradford Regional Medical Center campus, now known as Bradford Medical Park.
Physicians and advanced practice providers care for patients of all ages through annual wellness visits, preventive screenings, management of chronic conditions, same-day sick visits and coordination with Kaleida Health specialists when a higher level of care is needed.
Bradford providers include Dr. Jakub Wojcik, Danielle Gregoire, PA-C, and Jennifer Ursoy, PA-C. They continue to see patients in Suite 41 on the fourth floor of the outpatient services tower.
Dr. Cory Mathias and Sarah Schwab, CRNP, practice at 406 Franklin St. in Smethport. All providers are accepting new patients.
Primary care is often the first step in a patient’s healthcare journey. It is where long-term relationships are built, health concerns are identified early and personalized care plans are developed.
“Our providers know their patients beyond a single appointment,” said Jason Everett, clinical director of primary care. “These providers live and work in the community and they understand their medical history, their families and their health goals. Those relationships allow us to detect problems earlier, better manage chronic diseases and help patients stay healthier throughout every stage of life.”
Patients visiting the Bradford practices will continue to see the providers and care teams they know. The transition of the former Bradford Regional Medical Center into an outpatient services center has not changed its commitment to providing healthcare in the community.
Patients also have access to Epic, Kaleida Health’s electronic health record. Through MyChart, they can schedule appointments, communicate with their primary care teams, request prescription refills, review test results and access their health information in one location.
The connected record also allows providers across Kaleida Health to coordinate care more efficiently while maintaining personalized service.
“Strong communities begin with strong primary care,” said Bradley J. Chapman, president of Olean General Hospital. “When patients have access to a trusted primary care provider close to home, they’re more likely to receive preventive care, manage chronic conditions successfully and avoid more serious health issues. That’s why continuing to invest in primary care is one of the most important ways we can support the long-term health of Bradford.”
Studies have shown that people with primary care providers experience better health outcomes and receive more preventive care. Primary care emphasizes prevention, early detection, chronic disease management and coordination of care before health problems become more serious. Routinely seeing a primary care provider can improve life expectancy and reduce the risk of preventable hospitalizations.
“Healthcare still lives here in Bradford, it simply looks different than it did before,” Chapman said. “Primary care is the cornerstone of our outpatient services, and we are committed to providing as many healthcare services as possible right here in the community. When patients choose local care, they support the providers and caregivers who have served Bradford for generations and help strengthen the future of healthcare close to home.”
Other outpatient services provided by Kaleida Health in Bradford include pediatrics, women’s health, cardiology, orthopaedics and sports medicine, general surgery, urology, medical oncology consultations for new patients, pain management, physical medicine and rehabilitation, cardiac and pulmonary rehabilitation through Upbeat and occupational health services.
