UPMC Cole, Sweden Valley Manor announce partnership
COUDERSPORT — UPMC Cole and Sweden Valley Manor announced a strengthened partnership that will elevate long-term care services for the Coudersport community.
By early November, Cole Place, a skilled nursing facility located within UPMC Cole, will transfer its operations to Sweden Valley Manor, a skilled nursing facility located right across the street.
Residents and families can choose for residents to safely transition to Sweden Valley Manor’s campus at any time they feel ready or by early November. They will be welcomed into a warm, home-like atmosphere designed to foster comfort and community. Residents and families are also welcome to explore other long-term care options that best support their needs.

This collaboration between UPMC Cole and Sweden Valley Manor builds on our longstanding partnership in caring for Potter County and beyond.”
With its scenic setting and family-centered approach, residents will enjoy a more residential experience while continuing to receive the same trusted care they’ve come to expect from UPMC.
Cole Place staff will have the opportunity to join Sweden Valley Manor, preserving employment and ensuring familiar faces remain part of residents’ daily lives. This continuity supports a smooth transition and reinforces the strong bonds between caregivers and those they serve. This transfer of operations for Cole Place will not affect personal care home, Cole Manor.
